Medical Nutrition Therapy

Even if your insurance does not require a referral it is good to keep your doctor in the loop and to give Your Dietitian LLC access to your medical records. Please ask your doctor to complete this referral form and fax to 678-547-3156.

If you do not have insurance the cost is $160.00 per initial session and $80.00 per follow-up session.

Nutrition Education

Virtual supermarket tour and wellness call in office.

• $15.00 per person (minimum 5 persons)
• $10.00 per person (minium 8 persons)
